One of the most important components of our policies is our Client Health Intake Form. This form allows us to understand your unique health background, allergies, sensitivities, and any conditions that may affect the service — from skin concerns to chronic illnesses like GERD, autoimmune disorders, or sensitivities to certain ingredients. Nail care may seem purely cosmetic, but it involves direct interaction with your skin, natural nails, and sometimes your overall health. Knowing these details ensures that we choose the safest products, adjust techniques when needed, and avoid anything that could cause discomfort or harm.

This information is not just about precaution — it is about personalization. Understanding your health allows us to tailor your service with intention, ensuring the experience is both luxurious and medically mindful. (1) Services on client with infectious or contagious disease

(2) Operator with infectious or contagious disease performing services

(3) Services on inflamed skin or scalp without physician authorization

(4) Removal of corns, calluses, or other growths of skin by cutting

(5) Methyl methacrylate liquid monomer (MMA)

WHY WE ASK FOR YOUR HEALTH & MEDICATION INFORMATION:

MEDICATIONS AND UV SENSITIVITY

Some medications increase sensitivity to UV light, which can lead to irritation, burns, or nail conditions such as onycholysis (nail lifting).

Medications known to cause UV sensitivity include:

Antibiotics
Doxycycline, ciprofloxacin, sulfonamides, and tetracyclines can trigger phototoxic reactions or nail discoloration.

NSAIDs (Pain Relievers)
Ibuprofen, naproxen, diclofenac — can increase sunburn risk or blistering. Diclofenac is linked to onycholysis.

Acne Medications
Isotretinoin (Accutane) and tretinoin (Retin-A) may heighten UV sensitivity.

Antifungal Medications
Azole antifungals such as ketoconazole may cause photoallergic reactions.

Other Medications
Thiazide diuretics used for blood pressure can increase photosensitivity.

If you are taking any of these or similar medications, it is extremely important to let us know so we can adjust services safely.

DIABETES AND NAIL CARE

Clients living with diabetes require special attention during nail services.

Why is nail care so important with diabetes?

Reduced Blood Circulation
Healing slows, infections worsen quickly.

Neuropathy
Loss of sensation may hide injuries.

Higher Infection Risk
Even small wounds can escalate into complications.

Safe nail care tips for diabetic clients:

Inspect feet/nails regularly
Trim nails straight across
Moisturize (not between toes)
Wear properly fitting shoes
Maintain blood sugar
Seek professional help for thickened or painful nails

Avoid:
Cuticle cutting, walking barefoot, removing corns/calluses, ignoring infection signs.

If you have diabetes, please indicate this on your intake form so we can serve you safely.

NO FOOD, SMOKING, ALCOHOL OR DRUGS PERMITTED.

La Dolce Vita Nail Bar reserves the right to refuse service.

By receiving services, you acknowledge that all procedures are performed based on the information you provide and that withholding medical conditions, allergies, or sensitivities may increase your risk of reaction. La Dolce Vita Nail Bar is not liable for any medical expenses, treatments, injuries, or adverse reactions that may occur during or after your service. Any medical bills, follow-up care, or related costs resulting from a service or pre-existing condition are the sole responsibility of the client.

LA DOLCE VITA NAIL BAR PROPERTY DAMAGE / CLIENT RESPONSIBILITY

You are financially responsible for accidental or intentional damage to salon property. Items include (but are not limited to):

Nail polishes, gels, tools, equipment, furniture, electronics, décor, displays — any studio property.
La Dolce Vita Nail Bar is not responsible for lost or stolen property inside or outside the salon.
Please lock your car and avoid leaving valuables inside.

RESPECT AND TIMELINESS

Please arrive on time. You have a 10-minute grace period. After 10 minutes, your appointment may be canceled and a new deposit required.

CANCELLATIONS

Must be made 24 hours in advance.
Less than 24 hours or no-shows may incur charges.

Consultations (free) determine pricing and service design.
Nail services and deposits are non-refundable and cannot be exchanged.
Full payment is required before, during, or immediately after service.
Extra designs, charms, rhinestones, chrome, etc. incur additional charges.
Changes made on the appointment day may alter timing and pricing.

LA DOLCE VITA NAIL BAR CHILD POLICY & SALON ENVIRONMENT

We maintain a relaxing, adult-focused environment.
If you do not have childcare, please reschedule by contacting ciao@ladolcevitanailbar.com — we will work with your schedule. We completely understand.
Our building hosts other salons, so courtesy is essential.

GUESTS
One adult guest permitted if necessary (anxiety, support, etc.).

CHILDREN
No gel polish or enhancements for minors 17 and under unless a parent/guardian approves design and length.
Absolutely no gel or enhancements for children under 14.
Babies may accompany clients.

LA DOLCE VITA NAIL BAR RIGHT TO REFUSE SERVICE

We reserve the right to refuse service for reasons including:

Health concerns
Open wounds or contagious conditions
Disruptive behavior
Policy violations
Excessive lateness/no-shows
Unsafe or unrealistic requests

If a service must be stopped, payment may still be required for work completed.

LA DOLCE VITA NAIL BAR PHOTO, VIDEO & SURVEILLANCE POLICY

Photos/videos of nails may be taken for social media, marketing, website, educational, or safety purposes.
Your identifying features will not be shown.

A security camera operates for safety and may be used for legal purposes.

At La Dolce Vita Nail Bar, we accept the following forms of payment:

All card payments are securely processed through Square.

Please note: We do not accept personal checks. We also do not keep cash on the premises, therefore we are unable to provide change. We kindly ask that you bring a valid major credit card or be prepared to pay via Apple Cash for your appointment.
